Where are you located?

We have two locations:

This facility is home to our Adult Day and Memory Care Programs, some of our Behavioral Health team, and our administrative team.

This facility is home to our Physical, Occupational, & Speech Therapy team and some of our Behavioral Health team.

Is Independence Health & Therapy a Nonprofit?

Independence Health & Therapy is a tax exempt 501(c)(3) nonprofit corporation which pursues funding from local, State, and Federal sources as well as private foundations, corporations, and individual contributors. The clients we serve also provide fees to Independence Health & Therapy either directly, through private insurance, or other forms of coverage, as appropriate.

Does Independence Health & Therapy offer transportation?

Transportation services are a central component of all our programs. All clients, regardless of program, are eligible for door-to-door transportation.

What types of insurance do you accept?

We accept Medicare, Medicaid, private pay, and commercial insurance. We also accept the uninsured, offering scholarships and a sliding fee scale.

Do you provide services for caregivers?

We make a point of caring for caregivers. We offer a variety of services that provide respite for family members caring for loved ones. Other services provided are coaching, condition-specific education and workshops, and support groups. For more information, please call us at 815-338-3590.
