Family Alliance REACH Memory Care Program

Few programs exist that address the issues of dementia and also enable individuals to remain at home. Family Alliance is the only agency in our area that has the primary mission of enabling mature adults to remain in their home environment. Our REACH Memory Care program for persons experiencing memory loss includes a day program, in-home assessments, caregiver education, mentoring, and support groups.

The day program provides clients with activities based on their choice and abilities, while promoting physical and cognitive stimulation and valuable socialization with fellow clients and staff. Activities include small group and individual participation with craft projects, social activities, therapeutic music, animal assisted and horticultural therapy. Interest areas are set up throughout the dayroom to allow for individual participation and to encourage success by incorporating Montessori philosophies into the program model.

Services are provided by multidisciplinary team composed of registered nurses, certified nursing assistants, and activity specialists, all trained in dementia care. Click here to meet the full REACH Memory Care Team. Door to door transportation is provided throughout the county. A light breakfast and full lunch are included in the program.

Our REACH Memory Care program also provides valuable respite for family caregivers. For families who have concerns about a loved one who has recently been diagnosed with some form of dementia or who are experiencing challenges due to the symptoms of dementia, a Family Alliance registered nurse can partner with the family to help determine a course of care during an in-home assessment. In addition, our Licensed Clinical Social Workers will provide caregiver counseling upon request.

Caregiver mentoring and support groups are facilitated by individuals who have had experience caring for a loved one with dementia. There is one monthly group for all family caregivers and another monthly group for those caring for a spouse with dementia. Participants receive powerful and practical information and support from others who have learned through their own experiences. These are persons who have been or still are "walking the walk."
